Care Provider's Absence

The use of a substitute may be used in case of an emergency or for planned appointments that cannot be made outside of work hours.  The substitute must be over 18 years old, have a clear criminal record check, a clear child abuse registry, and valid first aid/CPR training.  For planned appointments, the care provider must have a Permission for Care Provider Absence form signed by the parents a minimum of 2 days prior.  In case of an emergency, the care provider must have an Emergency for Care Provider Absence form signed when the parents return to pick up the child.
